The Louisville double clamp has a frame with two cleats for securing stay sutures. In clinical practice, a double clamp without a frame or cleats is usually used and the assistant serves the function of holding tension on the stay sutures. Both clamps can be opened at once using the longitudinal groove in the end of the clamp applicator.

The artery is placed in the double clamps so that it lies one vessel diameter back from the tips of the clamps.

A straight forceps is placed across the center of the vessel, including the stump of the profunda. The scissors, coming from the opposite direction, excises the central segment immediately on either side of the forceps.
