A scissors cuts off the stretched cuff of adventitia flush with the end of the vessel. When it is cut, the remaining adventitia retracts 1mm back from the end of the vessel.

End to end Carrell (triangulation) anastomosis
As a descriptive convention to describe this anastomosis, imagine a clock face superimposed on the vessel end on the left. 12 o'clock is away from the operator, 6 o'clock is toward, 3 o'clock is down into the field and 9 o'clock is up toward the microscope.

The first stitch is placed (under high power) in the 11 o'clock position by inserting the tips of the forceps in the right side vessel end for support, with the blade opening at 11 o'clock. The stitch is passed outside to in one needle diameter from the edge, released by the needle driver, and gently teased out with the forceps.
