The front wall is completed by placing the 9 o'clock stitch. When first learning, it is wise to flip the clamp over before tying the 9 o'clock stitch to check that it has not caught the back wall.

The double clamp is carefully rotated to its back side around the axis of the vessel. The 3 o'clock stitch is placed and tied, leaving one end long as a lift stitch. This triangulates the vessel circumference into three 120 degree segments and separates the walls for ease of placing the remaining sutures.

The remaining two sutures are placed completing the anastomosis. A nice feature of the Carrell anastomosis is that multiple intermediate stitches can be placed for a larger diameter vessel.
