Once enough room has been made to introduce the Benson spreader, the spreader is used to split the fibers down to the mucosa from the antral end to the duodenal end. It can be used as shown here or by using the heel of the blades.

A hemostat can be substituted, but greater care must be taken to avoid perforating the duodenum. The tips of a hemostat must never be used to split the pylorus - use the heel only.

The fibers can be split onto the antrum with little worry. At the duodenal end, when the fibers are noted to be breaking at an angle to the incision instead of perpendicular to it, it is time to stop.
