A 13-day-old girl was brought to the operating room for excision of a sacrococcygeal teratoma. The teratoma had been diagnosed by ultrasound in utero. Early operation is important before malignant elements have a chance to spread.

The preoperative MRI showed minimal extension into the presacral space, making this a type I lesion with a low chance (8%) of malignancy. The higher the lesion, the greater the chance of malignancy.
