A 76-year-old woman presented with a history of three episodes of left sided amaurosis fugax. Duplex Doppler study showed high-grade stenoses at both the right and left internal carotid origins. In addition, angiographic study of the aortic arch (shown) demonstrated hi grade stenoses at the origins of the innominate and left common carotid arteries.

Because of the high risk of stroke, the patient was taken to the operating room for bilateral endarterectomy (see carotid endarterectomy) and bilateral aorto-internal carotid bypass. The field is draped for sternotomy and bilateral neck incisions.
