The skin edge is separated from the underlying cyst wall to start the plane of dissection. In areas of denser skin such as the back, or when there has been infection or inflammation, a clear plane is often not obtainable, and a rim of surrounding tissue must be removed with the cyst.

Dissection proceeds using a curved instrument like the hemostat shown, of a small Mezzenbaum scissors, following the contour of the cyst.

As the dissection deepens, a twisting motion is added to the spreading movement to free the circumference of the cyst.
