The skin is held gently with a toothed Adson forceps 1 cm from the site of the stitch. The needle enters the dermis at the dermal-epidermal junction, “one cell deep” and a solid 2-3 mm horizontal dermal bite is secured. The forceps manipulates the skin to direct the needle out at the same superficial level.

The site of penetration of the second stitch on the opposite side of the incision is determined by drawing the tail of the first stitch perpendicularly across the wound.
