Utilizing the commonly accepted safe margin for wide local excision of cutaneous malignancy of greater than 2 cm, the skin was incised down to subcutaneous tissue. The capsule of the axillary fat pad was opened at the margin of the skin paddle and a blue tinted lymph node with a reading over 1000 was identified as the first sentinel node.

Additional readings in the axillary fat pad identified four additional nodes with readings above 1000. Each was identified with silk sutures. The axillary contents ultimately removed comprised levels one and two.

A lead shield was used to isolate each node for confirmatory ex vivo counts. A final count of the remaining axillary contents did not identify any additional hot nodes. A 10mm closed suction drain was left. The subcutaneous tissue was approximated with 2-0 absorbable sutures and the skin was closed primarily with staples. The patient's arm was placed in a sling to avoid tension on the wound.
