The patient was taken to the operating room for extracranial-intracranial bypass (see also (see common carotid to internal carotid and external carotid to internal carotid). The patient was positioned to allow two teams to work simultaneously on harvesting the left radial artery conduit and exposing the left middle cerebral artery. The radial artery was chosen for size match and potential patency advantage.

Cerebral monitoring was established. Nitrous anesthesia was avoided due to the risk of air embolus and subsequent expansion of the nitrous into the air pocket.
