A 75-year-old man presented with obstructive jaundice and cholangitis. There were also symptoms of gastric outlet obstruction. Upper endoscopy revealed a carcinoma of the second portion of the duodenum obstructing the ampulla of Vater. The proximal end of the lesion is shown.
