Because the anastomosis could be localized in the right upper quadrant by pre-op X-ray studies, it was elected to avoid the midline mesh and approach through a limited sub-costal (Kocher) incision.

Multiple filmy adhesions were lysed and the anastomosis was exposed directly beneath the incision (viewed from the patientís left). The hepatic flexure was mobilized.
