The appendiceal mesentery is clamped, ligated and divided from distal to proximal, leaving a clear base.

A straight clamp is used to crush the base of the appendix and milk contents distally for a distance of a centimeter. A 2-0 absorbable suture is tied snugly at the base without strangulating the tissue. A second tie is placed at the distal end of the crush mark.

The appendix is cut off flush with the bottom of the straight clamp using a knife blade. The ends of the ties are left long for control of the stump.  Some surgeons cauterize the mucosa at the open end of the stump, theoretically to avoid mucocoele from viable mucosal cells.
