A 76-year-old woman presented after a syncopal episode. On physical exam, the blood pressure in her right arm (132/74) was lower than the pressure in her left (168/66). Angiographic arch study showed a high-grade atherosclerotic stenosis at the origin of the right subclavian artery.

The early phase injection shows filling of all vessels except the right subclavian and vertebral.

The late phase injection shows retrograde filling of the right subclavian via the right vertebral. Intracranial Doppler also showed reversal of flow.
