Gracilis Flap for Perineal Defect: 5

The gracilis anatomy and blood supply are shown schematically. The gracilis originates on the pubis near the symphysis and extends directly distally on the medial surface of the adductors to end on the medial surface of the proximal tibia. The blood supply arises from the medial femoral circumflex artery and from the profunda femoris.

Gracilis Flap for Perineal Defect: 6

Incision was made over the gracilis leaving a proximal skin paddle. The muscle was divided distally and mobilized proximally. Feeding vessels were divided up to the medial femoral circumflex pedicle. The anterior margin of the skin paddle was stapled to the adjacent skin for stability until the flap was ready to be transferred.
