A 38-year-old woman presented with a symptomatic right groin bulge. On examination, the bulge was mostly below the inguinal ligament. CT scan showed a homogenous cystic structure medial to the femoral vessels below the inguinal ligament.

The patient was taken to the operating room with a diagnosis of femoral hernia. An infrainguinal approach was planned (see femoral hernia anatomy and repairstrangluated femoral hernia, infrainguinal femoral hernia repair, and sigmoid in a femoral hernia) Given a choice of local, spinal or general anesthesia, the patient chose general. Ultrasound was performed prior to prep.
