A 23-year-old man presented with chest pressure and swelling of the left side of the neck. He reported fever, chills, night sweats and a 15-lb weight loss over the preceding three months. PA chest X-ray showed a widened mediastinum.

Lateral chest X-ray showed an anterior mediastinal mass. The preliminary diagnosis was lymphoma with a differential of thymoma, sub-sternal thyroid and germ cell tumor. Physical exam including testicular exam was unremarkable except for moderate diffuse swelling at the base of the neck on the left.

CT scan showed an 8.2 x 5.3 cm anterior mediastinal mass with tumor or clot occluding the superior vena cava (SVC) at the level of the pulmonary artery with 50% compression of the right pulmonary artery and mild narrowing of the right main-stem bronchus. The left brachiocephalic vein was occluded and there was a moderate pleural effusion.
