A 21-year-old man presented to the emergency room with a rigid abdomen, fever and elevated WBC after three days of vague abdominal symptoms. A diagnosis of perforated appendicitis was made on clinical grounds and the patient was taken urgently to the operating room. On introducing the laparoscope, inflamed omentum with fibrinous exudates was seen in the right lower quadrant.
