A 21-year-old man presented with sudden onset right testicular pain. Examination revealed an exquisitely tender, elevated right testicle.

The patient was taken immediately to the operating room with a clinical diagnosis of testicular torsion. The right testicle was exteriorized and the torsion visualized.

After detorsion the testicle appeared viable. Bilateral orchidopexy was performed and the patient recovered uneventfully. The total time from the onset of pain to detorsion was 4 hours.
