A 46-year-old woman presented to the emergency room with abdominal pain. Six months prior to admission she had a laparoscopic ventral hernia repair following multiple abdominal surgeries. Two months following repair she began having pain requiring narcotics for relief. Abdominal film showed a pattern of multiple tacks. She also had through-and-through musculofascial suture fixation of the mesh.

The patient was taken to the operating room for laparoscopic exploration of the mesh and fluid collection. A 10-mm port site opening was made in the right anterior axillary line. Space was made intraperitoneally using blunt finger dissection.
