A side-to-side stapled anastomosis is created between the proximal limb and the side of the distal limb 75-150cm distal to the site of division.

The Penrose drain is pulled from above the transverse mesocolon, drawing the free end of jejunum through in a retrocolic, ante-gastric position. The penrose is removed.

The end of the bypass limb is directed to the patient's left and a large enterotomy is created near the end of the limb with the cautery scissors.
