An endoscopic linear cutter is used to remove the free end of the jejunum, including the enterotomy. The gastrojejunostomy is reinforced with 3-0 silk sutures anteriorly, and the jejunal limb is sutured to the transverse mesocolon.  The anastomosis is tested by air and methylene blue die instillation. If a gastrograffin study on the first postoperative day shows no leak or obstruction, the patient is begun on a progressive diet.
