A 36-year-old woman presented with a painful bulge on the posterolateral aspect of her right brachium. The mass was soft on palpation with the triceps relaxed and fixed when the muscle was tensed, raising the suspicion that this was an intramuscular lipoma.

X-ray with the brachium externally rotated shows a smooth lucency close to the mid-humerus.
