The external carotid passes deep to the parotid gland where it divides into the internal maxillary and superficial temporal branches. Note the sternocleidomastoid branch of the occipital artery that descends over the hypoglossal sling and tethers it. This usually is divided to mobilize the nerve.

The stylohyoid muscle and posterior belly of the digastric cross lateral to the internal and external carotid arteries. The lower arc of the hypoglossal nerve is visible just below the digastric. It gives off the anterior root of the ansa hypoglossi supplying the strap muscles. The loop of the ansa passes anterior to the internal jugular vein (see below). The phrenic nerve spirals around the anterior scalene to enter the chest just lateral to the proximal common carotid. The proximal common carotid lies posterolateral to the lobe of the thyroid.

The large internal jugular vein descends from the jugular foramen to the root of the neck lateral to the carotid. It receives the large common facial vein over the carotid bifurcation, necessitating division of this branch to visualize the carotid.
