A 79-year-old man presented with abdominal distention, nausea and vomiting. The patient had no tenderness but had signs of ascites.

CT scan showed extensive loculated collections of dense homogeneous material. The patient was taken to the operating room with a presumptive diagnosis of pseudomyxoma peritonei.
