A 42-year-old woman presented with right upper quadrant pain, fever and a WBC of 17,000 three weeks following laparoscopic cholecystectomy. Her post-op course had been complicated by intraabdominal bleeding, which was treated non-operatively.

CT scan showed a localized fluid collection impinging on the posterior, inferior portion of the right lobe of the liver.

Although the cephalad portion of the collection appears partially intrahepatic, the proximity of the clips from the lap chole indicates the true origin of the collection. Note the air bubble in the abscess. The patient was started on broad spectrum antibiotics.
