The gastric contrast was interpreted as showing a leak. The patient was taken urgently to the operating room where the stomach was found to be normal. On mobilizing (Kocherizing, see Whipple procedure) the duodenum, a retroperitoneal perforation was found in the second (D2) portion. The hole was closed primarily and an omental patch was applied.
