The patient presented again with acute abdominal pain and distention. The abdominal film shown was obtained, showing a large loop of colon extending from the left lower quadrant.

CT scan shows the huge distended sigmoid with the point of torsion in the left lower quadrant. Compare these studies with the virtual colonoscopy reconstruction obtained two years before (see cecal bascule.) A diagnosis of sigmoid volvulus (see sigmoid volvulus, VID 994) was made. Rigid proctoscopy to 12 cm was unsuccessful at entering and decompressing the distended loop. Flexible sigmoidoscopy did achieve decompression.
