A 64-year-old man presented with progressive splenomegaly (see massive splenomegaly) associated with myelodysplastic syndrome. He was experiencing early satiety and abdominal discomfort. His platelet count was 50,000.

The spleen filled the left side of the abdomen and extended down below the level of the iliac crest.

The patient was taken to the operating room for elective splenectomy (see open splenectomy). The abdomen was markedly distended.
