The common carotid was exposed by opening the deep cervical fascia along the anterior border of the sternocleidomastoid muscle and dividing the common facial vein (see cervical neural tumor).  The superior thyroid artery, the first branch of the external carotid, was next isolated.

The hypoglossal nerve was located by following the ansa hypoglossi upward across the carotid bifurcation (see cervical anatomy). The occipital branch of the external carotid, which tethers the hypoglossal nerve downward, was divided and the nerve was moved cephalad out of harms way.

All vessels were isolated and encircled with vessel loops. Note the large size of the ansa hypoglossi. The area between internal and external carotid artery was not dissected, leaving the carotid sinus nerve intact. The posterior belly of the digastric muscle was dissected upward for additional exposure.
