Because of the negligible collateral circulation around this patient's circle of Willis, several precautions were taken to monitor neurological status during surgery. The procedure was done under local anesthesia with minimal sedation so the patient could reply to questions and move hands (with squeeze noisemaker) and toes. In addition, a continuous EEG monitor was used to detect slowing of brain waves indirectly indicating cerebral ischemia.
