A 42-year-old woman was discovered to have an incidental extraperitoneal mass inside the iliac crest on a CT scan done for upper abdominal wall pain. CT-guided needle biopsy showed a spindle cell neoplasm.

The patient was taken to the operating room for excision of the lesion. The retroperitoneal plane in the area of the right iliac fossa was entered through a right lower quadrant incision.

The lesion was exposed just inside the posterior aspect of the iliac crest. The lesion appeared round and uniform.
