The cord structures are peeled from the medial flap of external oblique with moist gauze wrapped around the surgeonís index finger. A clockwise rotation of the finger on the patientís right and counterclockwise on the right separates cord down to pubic tubercle.

The finger is rotated in the opposite direction to free the lateral side of the cord. These are avascular planes.

The ilioinguinal nerve is identified on the anterior aspect of the cremaster. A thin linear blood vessel running the length of the nerve helps identify it. The hypogastric branch of the iliohypogastric nerve can occasionally be seen on the surface of the internal iliac muscle cephalad to the spermatic cord.
