The first layer of the Shouldice sutures the falx to iliopubic tract and inguinal ligament with continuous 2-0 monofilament from pubic tubercle to deep ring. The second layer sutures the free edge of the medial/cephalad flap of transversalis down to inguinal ligament from deep ring to pubic tubercle, tying to the tail of the starting knot at the pubic tubercle (see hernia technique master class, frame 45.)
The McVay repair also starts by opening transversalis. A relaxing incision (see hernia technique master class, frame 54) is made in the deep layers of the anterior rectus sheath above the pubis to allow the falx to be sutured to the pectineal/Cooper’s ligament without tension.

As the femoral vessels are approached, the suture is passed from pectineal ligament through the medial edge of transversalis at the upper end of the femoral canal to the iliopubic tract and inguinal ligament, the transition stitch. The lateral portion of the repair takes the same layers as the iliopubic tract repair.
