The terminal branch of the IMA is the superior rectal artery which crosses the proximal left iliac vessels adjacent to the sacral promontory. Mobilization and division of this vessel is done close to the colon in benign disease in order to not damage the pelvic autonomic nerves and to not put the left ureter at risk. Greater care must be taken to avoid these structures when wider mesenteric resection is indicated as in total mesorectal resection for cancer, or when the mesentery is foreshortened from diverticulitis.

The right and transverse colon venous drainage is through the superior mesenteric vein which unites with the splenic vein to form the portal vein beneath the neck of the pancreas. the inferior mesenteric vein draining the left colon usually enters the splenic vein to the left of the fourth portion of the duodenum. Occasionally it enters the junction of SMV and splenic and rarely drains into the SMV.
