During a routine laparoscopic cholecystectomy, the cystic duct was isolated near the infundibulum, clipped and divided. Directly beneath the duct, there was a large caterpillar hump right hepatic artery (see Clinical Example A on previous page). One cystic artery branch has been clipped and divided (lower left), and another (top) has two of three clips placed prior to division. This anomaly is not uncommon (up to 15%) and puts the right hepatic artery at risk for injury.
