A 21-year-old woman presented with left upper quadrant pain and easy satiety. There was no history of unusual trauma or pancreatitis. Physical exam and labs were unremarkable. CT scan showed a cystic structure in the hilum of the spleen. The patient was taken to the operating room, placed in right lateral decubitus position and laparoscopic ports were placed. The first view of the cyst is shown.
