The anterior side of the axillary fat pad was dissected off the serratus and the long thoracic nerve was identified in its medial and posterior position.

All caudal branches of the axillary vein were ligated and divided, freeing the cephalad end of the fat pad laterally to the thoracodorsal pedicle.
