The flaps are identical in length and less than 90° from each other. They are situated at 45° to the corresponding defects they will transpose into. A Burow’s triangle is built into the modified design of the bilobed flap. The flaps are elliptical in shape to aid in closure.

Incisions are made along the flap outlines. The flaps are created with wide undermining to alleviate tension and avoid a pincushioning effect once the sutures are put in place.
