The lesion was excised with at least a 0.5 cm margin circumferentially. The tumor was marked and sent to pathology to check for margins.

The pathology report stated all margins were clear. However, the deep margin was less than 1 mm. The deep margin of the lesion was further excised, taking approximately an additional half centimeter of tissue, including part of the orbicularis oris muscle. This tissue was also sent to pathology for review.

A V-Y advancement flap lateral and inferior to the defect was created and advanced to the left ala and naris. The V-Y flap involves an incision in the shape of a V (or triangle shape), with the length to height ratio ranging from 2-4:1. 

The tissue on the sides is undermined, while avoiding undermining the pedicle. This allows for flap mobilization and maintenance of the blood supply through the dermal and subdermal tissues. The flap is then advanced in the direction of the defect, and sutured closed on all sides which creates the Y shape.
