A 37-year-old woman developed pseudocysts following a bout of alcohol related pancreatitis. The pseudocysts were treated by observation.

A year later the patient presented with sudden onset of sever upper abdominal pain. A pulsatile mass was present along with an audible bruit.

Aortogram showed extravasation along the course of the splenic artery. The splenic artery lies along the superior border of the pancreas (see spleen anatomy), and may lie in the posterior wall of a central pseudocyst. Digestion of the arterial wall by pancreatic enzymes can lead to the major complication of bleeding into the pseudocyst.
