The two 5mm right lateral ports are then placed. Counter pressure makes penetration easier and safer.

The anterior axillary line port is placed where there will be adequate reach to elevate the fundus well above the liver. The second 5mm port is placed midway between the umbilical and anterior axillary line ports, at the edge of the rectus muscle.

The surgeon, on the patient's left, uses the epigastric and para-rectus ports to operate bimanually (not depicted here). The assistant, on the patient's right, holds the fundus through the anterior axillary line port with the left hand, and the camera with the right hand. There are also robotic camera manipulators available.
