A 47-year-old man with a history of renal stones was found to be hyperparathyroid. He was referred from another institution after undergoing partial parathyroidectomy for hyperplasia.

CT scan of the head showed an "empty sella" suggestive of a pituitary cyst or a degenerated pituitary adenoma.

Abdominal CT scan revealed two pancreatic tumors, one in the neck and one in the tail. The tumor in the tail was more vascular than the one in the neck. On the basis of parathyroid, pituitary and pancreatic abnormalities, a diagnosis of multiple endocrine neoplasia, type I was made.
