There were extensive adhesions in the right upper quadrant from the patient's prior gallbladder surgery. The hepatoduodenal ligament was identified and the duodenum was Kocherized to control the head of the pancreas.

The gastrocolic omentum was opened below the gastroepiploic arcade and the lesser sac was entered.

The stomach was elevated off the body of the pancreas. The lesion in the proximal body of the pancreas was indistinct.
