A 58-year-old woman presented with blue toes, initially interpreted by her local MD as trauma. Subsequent examination by a vascular surgeon revealed no right femoral or distal pulses. The patient also reported buttock and thigh claudication. A diagnosis of blue toe syndrome was made and the patient underwent angiography.

Angiogram showed a high grade stenosis of the origin of the right common iliac artery. There was also a significant narrowing of the left iliac origin.
