A 45-year-old man presented with a three-month history of increasing weight loss, early satiety and post prandial stomach pain. This abdominal CT scan showed a 4.5 cm mass in the uncinate process  of the pancreas displacing the superior mesenteric vessels anteriorly. No lymphadenopathy was seen.

There was painless dilatation of the gallbladder (Courvoisier gallbladder) and bile ducts.

There was an ill-defined 1.5 cm mass in the periphery of the right hepatic lobe consistent with metastatic disease.
