A finger was carefully inserted anterior to the superior mesenteric vein from below and passed upward behind the pancreatic neck.

It is important to stay anterior and to the (patient's) left on the superior mesenteric vein, as there are several small branches draining the pancreatic head into the right side of the superior mesenteric/portal vein. The pancreatic neck was successfully freed, allowing commitment to proceed with a pancreaticoduodenectomy.

The gastroepiploic artery was ligated and divided on the greater curve of the stomach, and the stomach was divided between this point and the incisura on the lesser curve. A linear stapler with gastric length staples was used.
