If the DCIS is more diffuse (multifocal or multicentric) or if the patient cannot tolerate radiation, a total mastectomy may be chosen instead.

If the cancer is invasive but early, (i.e. stage I – see staging for details), breast conservation is most commonly chosen. However, because of the increased risk of spread to the axillary lymph nodes, surgical axillary sampling is added.

The newer modality of sentinel lymph node biopsy may eliminate the need for full axillary sampling in many cases. The greater the extent of axillary dissection, the greater the risk of postoperative arm swelling (lymphedema).
