The medial hilar pleura was opened.

The incision was extended down to divide the inferior pulmonary ligament. Freeing the inferior pulmonary ligament, the inferior extension of hilar pleura between the lower lobe and the mediastinum, later allows the lung to rotate upward and fill the chest.
