Because of the high risk of infarction from the multiple high grade stenoses, the patient was taken to the operating room for coronary artery bypass grafting. The legs and torso were prepped for vein harvest and bypass.

A sternotomy incision was made. The subxiphoid plane was entered at the lower end of the incision.
